There are sometimes problems in prescribing orthotic inserts. This is because they take up room in the footwear and with this condition, the foot is often already too large for most footwear.
Another problem with orthotic inserts is that the underlying heel block of the shoe or boot may not be wide enough to support the inner border of the orthotic which then rolls inwards making it ineffective.

Sometimes, for more dressy shoes which are for occasional evening use, we can make a shoe which accommodates and supports the foot without the use of an orthotic insert. This makes the shoe lighter, slimmer and more elegant.
Sometimes the main arch-supporting muscle can rupture.
One of the most successful ways of treating such a debilitating condition is to make a shoe or boot with full contact rear-foot support together with a strong wedge heel block under the inner border.
This can usually be built into the side so that it is not seen.
For light, summer, peep-toed sandals, an orthotic could be cumbersome.
